Before leaving for work or sending a student to school, conduct a wellness check to ensure good health. If you or anyone in your home is ill, please stay home. This tool is not meant to take the place of consultation with your healthcare provider or to diagnose or treat conditions. If you are in an emergency medical situation, call 911. These questions are designed to assess whether you should come to school or not.

  • In the last two weeks, did you care for or have close contact with someonediagnosed or suspected to have COVID-19? Close contact is defined as :
    • a person who provided care for the individual, including healthcare workers, family members or other caregivers, or who had other similar close physical contact without consistent and appropriate use of personal protective equipment OR
    • someone who lived with or otherwise had close prolonged contact for greater than 15 minutes (within six feet) with the person while they were infectious OR
    • anyone who has had direct contact with infectious bodily fluids of the person (e.g. was coughed or sneezed on) while not wearing recommended personal protective equipment OR
    • Contact occurred within the 48 hours prior to the infected person having symptoms OR prior to their positive test if asymptomatic

If you meet any of the above, you are requested to quarantine for 14 days from the last contact with the individual who has a positive test or is presumed positive for COVID-19.

  • In the last 48 hours, have you had any of the following symptoms:

  • Fever (100 degrees or +)
  • Chills
  • Cough
  • Headache
  • Muscle or body aches
  • Nausea, vomiting or diarrhea
  • New loss of taste or smell
  • New runny nose or congestion
  • Shortness of breath or difficulty breathing
  • Sore throat
  • Close contact with a person with COVID-19 in the last 14 days (cannot come to school or work)
  • Students must be fever free for 24 hours without the use of fever reducing medications in order to be at school.
  • Any symptomatic staff or student, including those presenting with just a fever, will be required to stay at home and may be asked to arrange to visit with their healthcare provider to determine if a COVID-19 test is appropriate. Staff shall contact their immediate supervisor and submit the appropriate leave documentation through Human Resources. Staff absences due to possible COVID-19 need to be reported to the Executive Director of Pupil Services.
